VBS TRANSPORTATION REQUEST & CONSENT FORM

Transportation is available for families who need assistance getting their children to and from Illumination Station VBS 2026. Our goal is to ensure every child who wants to attend has a safe and reliable way to participate. If you indicate on your VBS registration that your child needs transportation, please complete this Transportation Request & Consent Form. This information helps our Children’s Ministries team plan safe pick‑up and drop‑off routes, assign approved drivers, and communicate clearly with families throughout the week. All transportation is provided by background‑checked volunteers from Emmanuel Seventh‑day Adventist Church. Children will be transported following church safety guidelines and Texas state child‑passenger laws. Please complete one form per family and provide accurate pick‑up and drop‑off information.
